WSU VA Benefit Form
Enrollment Certification Request Form - Form required each semester for benefit certification for students. Students must
meet with their academic advisor to complete this form. *A new form must be submitted
for any enrollment changes.*

Military Leave of Absence (MLOA) Form - This is required for any servicemember requesting leave for military service. You
will also submit a copy of military orders as soon as they are available.
