institutional scholarship(s), you may appeal the withdrawal or non-renewal. This form
is to assist you in filing an appeal for institutional scholarships awarded through
the Office of Financial Aid only. Any scholarships awarded through your academic college
must be handled through that college's scholarship coordinator.
